Complete annual forms as soon as possible

Summer is always busy on campus as everyone gets ready for a new school year. Our school nurses are asking families to fill out their annual forms — which include health information — as soon as possible. 

To complete them, log in to the Parent Portal, then look for the Annual Forms link on the left menu of the Parent Dashboard.

These forms are easiest completed on a computer, not mobile device.

Athletes must complete OSSAA forms to be eligible

The OSSAA has transitioned all student-athlete eligibility forms to an online platform called Rank One. In order for your son or daughter to be eligible to participate in athletics this school year, both OSSAA Eligibility Forms must be completed through the Rank One system.

If your student is a returning Holland Hall student, the process should take only a few minutes.

If you are a new student to Holland Hall, complete the forms, and the forms will be sent to your previous school for release. The athletic department will monitor the process to ensure all new student forms are signed by the previous school and will follow up with any additional questions.

Getting Started

1. Click the link above.
2. If you are a first-time user, create an account using your email address and a password. You may also choose one of the alternative login options (Google, etc.).
3. Enter your student’s last name and Student ID number.

Finding Your Student ID Number

Your student’s ID number can be found in one of two places:

• On their Holland Hall ID badge.
• Through the Parent Portal (PCR):
  • Log in to PCR.
  • Select your student from the top menu.
  • Under Student Information, you will find the Student ID number next to your student’s name.

OSSAA Eligibility Form 1

1. Click Start Forms.
2. Select OSSAA Eligibility Form 1.
3. Select all sports your child will participate for the 2026-27 school year.
4. Confirm student information and enter your home address.
5. Answer Questions 1 and 2.
6. Scroll to the bottom and sign.
7. Click I Agree.
8. Click Submit.

OSSAA Eligibility Form 2

1. Click Back to Forms.
2. Select OSSAA Eligibility Form 2.
3. Complete the student information section.
4. Enter the following school information if requested:Last School Attended: Holland Hall (if your student attended Holland Hall last year)School Address:
   5666 East 81st Street
   Tulsa, OK 74137
5. Answer the eight eligibility questions.
   • Most families will answer “No” to all questions.
   • If you answer “Yes” to any question, please provide an explanation.
6. Student Signature:
   • Print name
   • Sign
   • Click I Agree
7. Parent Signature:
   • Print name
   • Sign
   • Click I Agree
8. Click Submit.

Final Verification

When you return to the Forms Management home page, both OSSAA Eligibility Form 1 and OSSAA Eligibility Form 2 should appear in green.

• Green = Eligibility forms are complete.
• Yellow = The form requires review by the Athletic Department.

If additional information is needed, we will contact you. Otherwise, once the forms have been reviewed and approved, your student will be eligible for participation.

Please do not complete any of the additional items listed below the Forms Management page, including NCSA or Personalized College Advising.
